* You should make an appointment at least 2 weeks before the ceremony date.

What you need to bring for marriage agent on appointment date:
1. Marriage License Application Form
2. Photo I.D. (Drivers License, Passport, State I.D. )
3. $65.00 Marriage License Fee in Cash to agent.
4. Receive the marriage license from agent.
5. Notify the agent that Sanae 'Sunny' Kamiya is the licensed wedding minister
that will perform your wedding ceremony.
What you need to bring on your Wedding day:
Present marriage license to Sunny at the ceremony.
(Don't forget to bring the rings)
The couple, marriage performer and witnesses will sign the marriage license form.
If only the couple is present at the ceremony, the marriage performer will be your
witness and sign the marriage license.
Please don't forget to submit "Marriage Records Request" as GREEN FORM.

How to obtain additional copies of the marriage certificate.
After the marriage:
Sunny will submit your marriage certificate to the Department of Health in
three business days after your wedding date.
A copy of pre-submitting marriage certificate is available upon request.
It will be emailed or mailed to you with your marriage license number.
It will take 4 to 8 weeks to receive your marriage certificate issued by
the State of Hawaii.
