What you need to bring for marriage agent on appointment date:
1. Marriage License Application Form
2. Photo I.D. (Drivers License, Passport, State I.D.)
3. $65.00 Marriage License Fee in Cash to agent.
4. Receive the marriage license from agent.
5. Notify the agent that Sanae 'Sunny' Kamiya is the licensed wedding minister
that will perform your wedding ceremony.
What you need to bring on your wedding day:
Present marriage license to Sunny at the ceremony.
(Don't forget to bring the rings)
The couple and marriage will sign the marriage license form.
After the wedding ceremony:
Sunny will submit your marriage certificate to the Department of Health in
three business days after your wedding date.
A copy of pre-submitting marriage certificate is available upon request.
It will be emailed or mailed to you with your marriage license number.
1. Regular mail: 6 to 8 weeks for delivery
2. USPS priority mail with a prepaid self returned envelope: less than 6 weeks.
Note: The return time for the certificate may be extended due to high volume
of requests and also due to record search to follow up your certificate.
